You are hereby commanded to appear before the Judge of said Court at the Putnam Circuit Court, 1 Courthouse Square, Greencastle, Indiana, (telephone: 765-653-5315) on November 17, 2016 at 1:30 P.M. to attend an Initial hearing/Termination hearing and to answer the Petition for Termination of your Parental Rights in said Child.
